Volunteer Agreement

Whereas it is agreed: Companion Pet Rescue, hereinafter referred to as CPR, and the Volunteer listed below are desirous of entering into a Volunteer Agreement to promote the humane and proper care of a rescued animal. CPR has been established to provide for the rescue and care of animals who have been abandoned, neglected and/or abused by their previous owners.

I understand that CPR will determine the criteria for becoming a volunteer. If I am approved as a volunteer, I may ask that CPR find another volunteer to take over the care of an animal in my possession, either by my decision or CPR. I understand that I will be responsible to keep CPR informed of any medical or emotional conditions that the animal may have, based upon information provided by the former owner or animal shelter/humane society. I understand that as a volunteer, I am expected to keep the animal secure and to care for it humanely. The Volunteer represents that no member of the family has ever been charged with cruelty to animals.

CPR retains ownership of all rescued animals until placed in their permanent home. I understand that an animal may need to occasionally be euthanized if it becomes sick or exhibits behavior problems making the animal unsuitable for placement as a companion animal.

I, ___________________________, hereby agree to abide by the following terms during the time I am a volunteer for CPR:

1. I will treat all animals as a responsible and humane pet owner.  I will provide said animal with all care and attention necessary to ensure its health and well-being. This will include providing sufficient food, water, exercise, competent veterinary care, annual shots, worming and prompt veterinary care in the event of illness or injury.

2. I will not use any animal that comes to me through CPR for breeding purposes of my own or allow it to be so used by any other person.

3. I will remember in all my dealings with the public that I represent CPR. 

4. I agree that any animal in my care remains the possession and property of CPR until that animal is placed into a permanent home.

5. I acknowledge that my residence, if rented, permits pets pursuant to the lease or by special permission of the landlord or resident manager.

6. I agree to allow inspection of my home and the conditions in which the animal is living at any time by a representative of CPR. I also agree to immediately surrender custody of the animal to a CPR representative upon demand.

I have read and understand all of the statements above. I understand that any contact I have with an animal as a CPR volunteer, is done at my own risk. If I am selected as a volunteer, I can ask at any time for an animal to be put with another volunteer that may be more suited to handle its problems. As a volunteer for CPR, I acknowledge that CPR, nor any of it's board members, is not responsible for any property damage or personal injury suffered by me, members of my household, third parties or my other pets by any CPR animal whom I come into contact with as a volunteer, and I assume all liability for providing adequate controls to prevent such damage or injury.

I accept full responsibility for any and all expenses incurred during my tenure as a volunteer and representative for CPR, except for those pre-approved by CPR. I further agree to keep records of all animals put into my care such as, release and adoption forms, monies collected and expenses incurred for each and every CPR animal. I further agree that such records are the property of CPR and that a representative thereof may request to view and/or obtain these records at any time. In the event I resign as a volunteer, whether voluntarily or by request of CPR, I hereby agree to return any animal in my possession and all CPR animal records over to CPR within ten days of resignation of request date.

I have accurately completed the Volunteer Application and Agreement and I have read the above warnings and appreciate the risks involved in working with rescued animals. I understand that CPR cannot be responsible for the actions, behaviors, and/or medical condition of the animals that it seeks to rescue, and I agree to assume the risks implicit in working with rescued animals which may have been abandoned, beaten or otherwise mistreated or abused, or who may suffer from an illness, condition or disease.
